Strides CREST Program Referral Form
The CREST (Community Response to Exploitation, Sex work & Trafficking) team provides mental health and case management support that is based in the principles of trauma-informed care and collaboration. We work with individuals who are between the ages of 12–29 and who may have a range of experiences with various forms of sexual exploitation or sex trafficking. We also support young adults aged 18–29 who have been, or are involved in, sex work by choice, circumstance or coercion.
This referral form is used by our team to better understand your needs and how our program may be a fit for you. Some of the questions are personal in nature, and we understand that answering them may bring up uncomfortable feelings. The information you provide here is important, as it helps us to pair you with an appropriate primary worker who will best suit your needs and/or for us to provide comprehensive and relevant referral information for you.
The information you choose to share with us is confidential, meaning that we will store your information securely and not use or disclose your information outside of providing you with service. However, there are some situations when confidentiality may need to be broken due to the risk of harm. If situations like this are brought to our attention, we will inform you of our duty to report and include you in the process as collaboratively as possible.
Our program is not set up to provide crisis counselling or support. However, there are resources available to support you if your need is urgent. If you are in immediate danger, please contact 911. If you are not in immediate danger, please see this list of resources:
